Before arriving for your appointment, please make sure you have:

  • Your insurance card
  • A photo ID
  • A list of all current medications
  • Reports and films/CDs of any MRI or CT scans

Upon your arrival:
The front desk will make copies of your insurance card and ID and verify that all information in our database is accurate. They will then provide all necessary forms for you to complete and sign, and finally, they will collect your copayment.

Underage patients:
A parent or legal guardian must accompany new patients under the age of 18 to their initial visit. A parent or legal guardian may designate another adult to accompany an existing patient during follow-up visits if this is specified on a Minor’s Consent for Treatment form. This form can be obtained during check-in.

Appointment timing:
We always do our very best to meet your appointment time as closely as possible. However, our therapists may need to spend a little more time than anticipated with other patients. Please understand that we will give the same level of care to you. For these reasons, we appreciate your patience and apologize for any delays you may experience.

Please be sure to arrive on time for your appointment. If you need to cancel or expect to be late, please call us as soon as possible so our therapists can adjust their schedules, since they may be with other patients.
